There are two different visa applications acceptable at the Russian Consulate in New York, NY based on the country of your citizenship.

If you are US citizen you should complete detailed two page visa application for US citizens. (linked to PDF)

For all other applicants there is one page visa applications for Non-US citizens. (linked to PDF)

You may complete the visa application using the PDF Typewriter tool or print out the form and complete it by hand. Both options are acceptable by the consulate.

Please, make sure you complete all questions, in order to avoid the delay of your Russian visa processing. If you have any question, please, contact your travel agent, if you using certified agent service, or leave the question blank and consult consulate officer if you are applying in person, try to avoid making corrections on your application as much as possible.

Thinks you need to remember while completing the Russian visa application:

1. Make sure you have put the correct passport information, passport number, date of issue/expiration; your name should appear exactly as it is stated in your passport!
2. The dates of entrance and departure should be the same is on your visa support letter: tourist voucher with confirmation or original business / cultural visa invitation issued by the Ministry of Foreign Affairs of the Russian Federation. Also, make sure that the spelling of your last and first name, the date of birth and passport number are matching your current passport information. In case of you have to renew your passport and the invitation was issued based on your expired/lost passport, you need to provide the copy of your previous passport along with the invitation.
3. Medical Travel Insurance is a must. For US citizens: write the name of your local insurance, no copy required. For Non-US citizens: you have to provide the copy of your medical insurance and the telephone number of your insurance company participating party on the territory of Russia.
4. In case if you have never been employed or studied, etc. you should put NONE or N/A in the relevant field.
5. If you have ever been arrested or convicted for any offence within the past ten years you should provide the copy of the letter of disposition indicated that all charges have been dropt or you have completed all the penalties.
6. You place of residence while travelling to Russia: If you are applying for tourist visa you should put the information of the hotels indicated in your tourist voucher. If you are applying for business/cultural visa you may indicate the hotel or residence address where you are planning to stay or in case if you have not made any reservations – indicate the address of the inviting organization.
7. Current place of work: if you are currently self employed, please indicate the name your registered business if there is one or simply put “self-employed” in question #40 and your current residence address.
8. IMPORTANT: you have to put the date and sign the completed application.
